Meet some of the team members who use their expertise and servant leadership to help Lake Forest Place thrive.
Anthony Madl
Executive Director
Anthony Madl was named Executive Director of Lake Forest Place in 2023. He has more than 24 years of experience in senior living operations leadership, including 17 years as Nursing Home Administrator and over seven years as an Executive Director. He is responsible for operations management, organizational change, strategic planning, resident engagement and delivering financial results.
Prior to joining Lake Forest Place, Madl was Executive Director at Greenfields of Geneva. He joined Greenfields of Geneva in 2018 and provided leadership in the areas of operations management, organizational change, leadership development and census development. Through his leadership, the organization targeted marketing efforts that boosted post-COVID sales and played a key role in the ownership transition for both residents and employees.
He began his professional career at Resurrection Health Care in 2001 where he operated 5 of their 11 communities ranging from their highest acuity level community to their largest community (298 units). Madl then joined Friendship Senior Options as Administrator in 2012 before promotion to Executive Director at Greenfields of Geneva in 2018.
Born and raised in Des Plaines Illinois, Madl obtained a Bachelor of Science in Sociology, Health and Aging from Northern Illinois University and a Master of Business Administration from the Keller Graduate School of Management. He is a licensed nursing home administrator and has served as President and an Executive Board Member of the Niles Chamber of Commerce.
Anthony Rizzato
Community Life Administrator
Anthony Rizzato was named Community Life Administrator of Lake Forest Place in 2021. He has over 30 years of experience in aging and human service leadership. Anthony is responsible for ensuring uninterrupted services for Independent Living residents through 12 service lines of business in Community Life and 100 dedicated associates. Anthony provides daily operational oversight of programs assigned and actively supports strong hospitality engagement, strategic planning, and program development.
Prior to joining Lake Forest Place, Anthony held progressively responsible roles as Director of Social Work, Healthcare Administrator and Executive Director at regional Life Plan communities in Illinois.
Anthony began his professional career at Catholic Charities of the Archdiocese of Chicago where he was promoted to Department Director of the Central States Institute (CSI), a program that provided intervention services within the six Municipal Circuit Courts of Cook County, Illinois. He has presented to the professional community on a variety of societal topics on the local, state and national level and served on numerous committees.
Born and raised in the Chicagoland area, Anthony obtained a bachelor’s degree in science, Criminal Justice from Loyola University of Chicago and holds master’s degrees in social work, also from Loyola University and a master’s degree in healthcare administration from the school of business at National Louis University. He is a Certified Dementia Practitioner, Licensed Social Worker, Licensed Nursing Home Administrator and a graduate of the LeadingAge Leadership Academy and recipient of leadership skills development from the Pivotal group in Canada.
Beth Welch
Senior Director of Community Programs and VolunteersBeth Welch, Senior Director of Community Programs and Volunteers, has worked for Lake Forest Place since 2018. Along with her team, Beth oversees the planning of parties, concerts, lectures, classes, bus trips and more. She supports our Resident Special Interest Groups, where residents connect over shared expertise and interests. In 2023, she founded Club Unity, a supplementary activity program for independent living residents who have early-stage memory loss.
Beth has 30 years of experience in planning programs for seniors. Prior to joining Lake Forest Place, she worked for similar non-profit, senior living and care organizations in all levels of care including Independent Living, Assisted Living, Skilled Nursing and Adult Day Services. In these positions, she learned each community’s unique preferences and interests to create and implement programs that enhanced quality of life as it is uniquely defined by individual residents.
Beth’s journey in senior living began as a teenager, working as a waitress at a retirement community in Michigan. She went on to earn her bachelor’s degree from Western Michigan University, majoring in recreation with a minor in gerontology. During college, Beth gained hands-on experience through part-time roles in home care, a geriatric psychiatry unit at a rural hospital, and internships in senior living communities.
After completing her undergraduate studies, Beth moved to Chicago to pursue a master’s degree in gerontology at Northeastern Illinois University. Over the years, she has shared her expertise by presenting at industry conferences and teaching classes through LeadingAge and the American Society on Aging.
Carol Van Berkel
Health Care Administrator
Carol Van Berkel was named Health Care Administrator of Lake Forest Place in January of 2022. She has more than 12 years of experience in senior living leadership across all levels of care. She is responsible for the daily operations at Balmoral Care Center and the Highlands Assisted Living at Lake Forest Place. She ensures compliance with state and federal regulations and oversees the budget for the department.
Prior to joining Lake Forest Place, Carol was the Independent Living Social Worker at The Moorings of Arlington Heights. She joined The Moorings in 2016 and provided leadership in wellness interviews for incoming residents, transitions through the Life Plan Community and provided educational programs for independent living residents in collaboration with other departments.
Carol began her professional career in senior living leadership at Sunrise Senior Living in 2012 where she worked as the Memory Care Coordinator and she has also worked as an Assisted Living Director.
Born and raised in Minnesota, he obtained a bachelor’s degree in social work from Dordt University in Sioux Center, Iowa. She is a Licensed Nursing Home Administrator and a Certified Dementia Practitioner.
Ceazar Familara
Director of Nursing at Balmoral Care Center
Ceazar Familara was named Director of Nursing at Balmoral Care Center in 2022. He brings more than 28 years of leadership experience in nursing and long-term care. He is responsible for supervising nursing staff, setting and implementing clinical goals, ensuring regulatory compliance, and maintaining the highest standards of patient care.
Prior to joining Balmoral Care Center, Familara served as a Wound Care Nurse at Radford Green at Sedgebrook beginning in 2009. There, he led initiatives in clinical quality, care planning, and wound prevention. Under his leadership, the community developed and implemented individualized care plans that significantly improved wound healing outcomes and reduced pressure injuries.
Born and raised on the island of Oriental Mindoro, Philippines, Familara earned a Bachelor of Science in Nursing from St. Jude College of Nursing in Manila. He passed the Philippine Board of Nursing in 1996 and later that year passed the Commission on Graduates of Foreign Nursing Schools (CGFNS) examination. In 1997, he became licensed in Illinois and began practicing as a nurse in the U.S. He is Wound Care Certified by the National Alliance of Wound Care and Ostomy (NAWCO) and is a certified Infection Preventionist through the Association for Professionals in Infection Control and Epidemiology (APIC).
John Janco
Senior Director of Sales and Marketing
John Janco was named Senior Director of Sales and Marketing of Lake Forest Place in 2024. He has more than 30 years of experience in hospitality and senior living leadership. He leads the team responsible for guiding prospective residents through the decision-making process at Lake Forest Place.
John began his career in 1985 at The Palmer House Hilton Hotel, working in reception while attending college. After graduating, he pursued a career in hospitality sales and marketing, advancing into leadership roles and relocating across the United States. Before his current position, John held sales and marketing leadership roles at several senior living communities in the Chicagoland area.
John has lived in Houston, Las Vegas, Dallas, and Saugatuck, and is grateful to be back home in Chicago. Born and raised in Chicago, John obtained a Bachelor of Science in Marketing from The University of Illinois – Chicago.
Andrew Rosencrans
Chaplain and Director of Spiritual Life
Rev. Dr. Andrew Rosencrans has been the Chaplain and Director of Spiritual Life at Lake Forest Place since October 2008.
Dr. Rosencrans has more than 35 years of experience as a preacher, teacher, theologian, and provider of pastoral care. He is responsible for the management and leadership of Spiritual Life, including officiating services of worship, teaching educational programs, overseeing memorial services, moderating support groups, and providing pastoral care.
Prior to being called to Lake Forest Place, Dr. Rosencrans served for 15 years as the Pastor of the Presbyterian Church of Llanerch in Havertown, Pennsylvania; and for four years as the Pastor of the St. Andrew’s Presbyterian Church in Sanford, North Carolina.
Born and raised in Pittsburgh, Pennsylvania, Andrew’s educational background includes a Doctor of Ministry degree from McCormick Theological Seminary; a Master of Divinity degree from Princeton Theological Seminary; an MBA from Carnegie-Mellon University; and an AB from Kenyon College.
His ordination as a Minister of the Word & Sacrament is through the Presbyterian Church (USA).
